 CAP Form 50

(CAPF50-1, 50-2, 50-3, 50-4)


1) A “Form 50” must be filled out in advance for each cadet who desires a promotion board.


2) The form changed effective Oct. 2006:

         a) The "Cadet Progression Evaluation" (old version F50) is     obsolete.

         b) The new forms are titled Cadet Leadership Feedback

         c) Each phase of the cadet program now has its own version of the F50 that you can download here, to be used as follows:

   

F50-1 is for use during Phase I

        Curry Achievement through Wright Brothers Award (Achievements 1-3) C/Basic to C/SSgt

F50-2 is for use during Phase II

        Rickenbacker Achievement through Mitchell Award (Achievements 4-8) C/SSgt to C/2Lt

F50-3 is for use during Phase III

        Flight Commander Achievement through Earhart Award (Achvmt’s 9-11) C/2Lt to C/Capt

F50-4 is for use during Phase IV

        Leadership Officer Achievement through Eaker Award (Achvmt’s 12-16) C/Capt to C/LtCol

Files ending with ".dot" can be opened with Microsoft Word, with the advantage that they can be filled out on your computer, and then printed with your notes.

 

Files ending with ".pdf" can be opened with Acrobat Reader.  Data cannot be entered on this form via computer, but it can be printed then filled out manually. To download the latest version of Acrobat Reader, go to Adobe's website: http://www.adobe.com/products/acrobat/readstep2.html
